Dukinfield Medical Practice came into being in July 2016 when the Davaar Medical Centre and the Manor House Surgery merged to form a new practice.
At Dukinfield Medical Practice we offer a range of appointment types:

Repeat Prescriptions
All prescriptions will be sent electronically to your nominated pharmacy within 48 working hours.
To order a repeat prescription, please complete the online form on our website or, alternatively, you can telephone the surgery.
Medical Records
You can request a copy of your medical records by completing the online form on our website or by writing to the surgery.
Please note that there may be a charge for this service.
